Wisdom Tooth Removal: Cost, Recovery and What to Expect
Not every wisdom tooth needs to come out. For the ones that do, here is the honest version of the cost and the recovery.
Wisdom teeth arrive between 17 and 25, in a jaw that usually has no room for them. Most people will be told at some point that theirs should come out. Some of them are right.
When removal is actually needed
- Recurrent pericoronitis — the gum flap over a partly erupted tooth traps food and keeps getting infected.
- Decay in the wisdom tooth or, more importantly, in the second molar in front of it, which is very hard to clean and easy to lose.
- Cyst formation around an impacted tooth, seen on X-ray.
- It's damaging the neighbouring tooth by pressing into its root.
When it's reasonable to leave it
A fully erupted wisdom tooth that bites properly, is cleanable and has never caused trouble does not need removing for its own sake. Prophylactic removal of asymptomatic, fully erupted wisdom teeth is not routinely recommended. If you're being advised to remove all four "just in case" with no symptoms and no X-ray finding, a second opinion is fair.

What happens on the day
Local anaesthetic, so you're awake and shouldn't feel pain — pressure and sound, yes. A simple extraction takes 10–20 minutes. A surgical one involves a small gum incision, sometimes removing a little bone, sometimes cutting the tooth into pieces to bring it out through a smaller opening, then a stitch or two. Typically 30–45 minutes.
Recovery, day by day
- Day 0: Bite on gauze for 30–45 minutes. Ice pack, 15 minutes on and off. No spitting, no rinsing, no straws, no smoking — all of these can dislodge the clot.
- Days 1–2: Peak swelling. Soft, cool food. Start warm salt-water rinses only after 24 hours, gently.
- Days 3–4: Swelling starts to fall. Jaw stiffness is normal and common.
- Day 7: Stitches out if they aren't dissolvable. Most people are back to normal eating.
- Weeks 2–4: The socket fills in. Full bone healing takes months but you won't notice it.
Dry socket — the complication to know about
If the blood clot is lost from the socket, the bone is exposed and it is genuinely painful, usually starting three to four days after the extraction. It's more common in smokers and after difficult lower extractions. The prevention is unglamorous: don't smoke, don't use straws, don't rinse vigorously for 24 hours. If a severe, throbbing ache begins on day three and painkillers aren't touching it, go back — a medicated dressing settles it quickly.

- Pain that worsens after day three instead of improving
- Fever, or swelling that spreads to the eye or under the jaw
- Difficulty swallowing or opening your mouth more than a finger's width

Frequently asked questions
How long does it take to recover from wisdom tooth removal?
Most people return to normal daily activity within two to three days and to normal eating within a week. A difficult bony impaction can take ten to fourteen days for the soft tissue to settle fully. Plan for two quiet days immediately after, especially for a lower surgical extraction.
Should I remove all four wisdom teeth at once?
It is common to remove two on the same side in one sitting so you can chew on the other. All four at once is done under sedation or general anaesthetic in some cases, which raises cost and recovery difficulty. Discuss both approaches — there is no single correct answer.
Can I go to work the day after wisdom tooth extraction?
After a simple extraction, usually yes. After a surgical extraction of an impacted lower tooth, take at least one day off — swelling peaks around 48 hours and jaw stiffness makes talking uncomfortable.
